PAYMENT AND COLLECTION OF CHARGES AND FARES: Payment and collection of carriers' fares and charges shall be by credit card at time of booking service or cash at the time of pick up.

TAXES: GENERAL EXCISE TAX (GET) will be added to all fares



Special Service Transportation Policies:

1. A.  This is door-to-door transport from the participant’s home to medical facilities or any other appointments. and/or return. It includes wheelchair transport, and necessary transfers from wheelchair to van. Participants are on a scheduled list for pick up and drop off. If there are changes, they are required to call in ahead of scheduled pickup or drop-off. They must be ready for pickup scheduled time. Drivers are not personal assistants, and when such service is required, an additional attendant will be applied at the rates indicated below
   


2. B.  Staff are regulated by all state Department of Labor and Human Services regulations. Staff pick up and drop off at the door, they are not allowed to enter the home to assist in any personal preparations. Staff must always demonstrate safe driving skills and appropriate care and concern for all participants.


3. C.  Charges are computed on Fees for type of service, plus Milage for distance of service, and when applicable Waiting Time for standing by.


4. D.  Standard service hours are as follows: Column 1, Hours from Monday through Friday between 8:00 a.m. and 5:00 p.m. except as show in Column 3. Column 2, Hours other than shown in Column 1 from Monday through Friday and Weekends, except as shown in Column 3. Column 3, All Federal holidays.

Application:

1. A.  A driver is included for each vehicle for which a charter fare is quoted in the schedule above.  If wheelchair service or additional attendanty is required or requested, then fees as shown in Item 210 will apply. 
   


2. B.  There will be a minimum charge of two hours for every charter.


3. C.  A fractional hour for (lver 2 hours) shall be when the time involved is less than 30 minutes, the charge shall be for 30 minutes.  When in excess of 30 minutes, the charge will be for one hour.


4. D.  These fees apply to any reservations for charter, exclusive cars, or F.I.T.'s as provided in this section.  Cancellation fees are determined as follows:
