What is the role of a concierge care coordinator ?

I am educated and trained to coordinate and aid clients and their families with challenges such as housing, home care services, medical management, and legal and financial issues. I will function as a liaison between the client, family and other professionals while providing a client-centered approach to care coordination.

I am independent of any service agency so there are no overhead systems governing what can and can’t be addressed. Does my insurance cover these services?

These services are private pay. I am an expert in the navigation of insurance benefits. Let me explore the benefits you may qualify for and are not utilizing. Care management services are not currently covered by medical insurance, however, if you have a long-term care policy, we can explore some of these options available. Health insurance will often pay for short-term interventions through home health agencies. These agencies provide skilled nursing, physical therapy, occupational therapy, social services, speech therapy, and home health aides.

Are there free care coordination services available?

Yes. There are government and community-based programs that provide some forms of support. However, these can be limited, fragmented and uncoordinated- and many clients fall through the cracks. Investing in an independent Care Manager ensures that your loved one has personalized care coordination during every interaction.

How do I find out more about your services?

Many other companies offer care management services as a part of their portfolio of services, which includes in-home caregiving. I DON’T, AND THIS IS WHY. As you might imagine,  there is a huge need for caregiving services, these companies will only present the option of collaborating with their company. What if their company is offering sub-par support? I take pride in avoiding potential conflicts of interest, and continuously liaise with a multitude of caregiver and support services agencies in the area. Because I am an independent care management company, I am confident that my recommendations will be just the right fit for your situation. I believe that is why so many physicians, healthcare professionals, and elder law attorneys recommend my services. You will get an honest, unbiased assessment and a recommendation for continuing services you can trust.
